These free medication list and health log templates give you 16 Word documents on US Letter and A4: a medication list, a medication log, a blood sugar log and a daily food log, each in color and in black and white. I’m Patrick Elliott, a teacher and Inclusion Lead. I built these so a medication list fits on one page with clear columns, and the daily logs are quick to fill in and easy to read at an appointment.
In this guide, you’ll find which log suits what you need to track, what to write in each column, and how to keep your records ready for appointments.

What Is a Medication List?
A medication list records every medicine you take: its name, the dose, how often, what it is for and who prescribed it. Carrying an up-to-date list helps doctors, pharmacists and family members, especially in an emergency or when you see someone new. The logs in this set record what you take day to day, your blood sugar readings and what you eat.
Which Health Log Should You Use?
- Medication list: each medicine with its dose, how often, what it is for, the prescriber and the start date, plus allergies and your pharmacy.
- Medication log: the date, time, medicine and dose, with a check box for each dose taken.
- Blood sugar log: readings before and after breakfast, lunch and dinner and at bedtime, with notes.
- Daily food log: meals and snacks with the time, what you ate and drank, the amount, and eight water glasses to check off.
- Black and white versions: every template also comes in a printer-friendly black and white version with grey headings, to save ink.
How to Use a Medication List and Health Logs
- Type your medicines into the medication list, including vitamins and over-the-counter medicines, then print a copy for your wallet or bag.
- Check off each dose on the medication log as you take it.
- Write blood sugar readings in the right column straight after you test.
- Take your logs to appointments and update the list whenever a medicine changes.
These logs are for keeping records only: they give no target ranges or medical advice. Write the units your meter uses at the top of the blood sugar log, and ask your doctor or nurse what your readings mean for you.
Black and White Versions
Every template on this page also comes in a black and white version with the same layout: grey headings and an outlined title, so it prints clearly on any printer and saves ink. In the download box, choose Colour, Black and white or All, then your paper size: A4, US Letter or both.
How to Print at the Right Size
Print at 100% on US Letter or A4. Keep a printed medication list in your wallet or bag, and print fresh logs each week or month.
