These free behavior chart templates give you 10 Word documents on A4 and US Letter: a clip-up chart, a traffic light self-check, an individual weekly chart, an ABC data sheet and a frequency data sheet. I’m Patrick Elliott, a teacher and Inclusion Lead. As an Inclusion Lead I plan behaviour support every week, so I made charts that only move children up, a traffic light with calming strategies, and the ABC and frequency sheets I use to find patterns.
In this guide, you’ll find which chart suits your class or one learner, how to keep a behaviour chart positive, and how to record ABC data that helps you plan.

What Is a Behavior Chart?
A behavior chart, or behaviour chart in the UK, helps a child or a class see how they are doing and what to do next. The best charts are positive: they notice what goes well, teach a strategy for hard moments and never shame a child in front of others. Data sheets are different: ABC and frequency records help an adult see patterns, such as what happens just before a behaviour, so support can be planned.
Which Behavior Chart Should You Use?
- Clip-up chart: everyone starts on Ready to learn and can only move up, for whole-class praise.
- Traffic light self-check: a child points to green, amber or red, then tries a calming strategy and their own plan.
- Individual weekly chart: three goals ticked each day, with a reward agreed with the child.
- ABC data sheet: antecedent, behaviour and consequence notes, for spotting what triggers a behaviour.
- Frequency data sheet: a tally for each session across the week, to see when a behaviour happens most.
How to Use a Behavior Chart
- Choose one or two clear, positive goals and say them in the child’s words.
- Agree the strategies and the reward with the child before you start.
- Check in at the same times each day and notice what went well first.
- Keep personal charts private, and review the data each week with the people who support the child.
Use the ABC and frequency sheets for a week or two before you change anything: the pattern is often clearer on paper. Share the traffic light chart with home so the same words and strategies are used everywhere.
How to Print at the Right Size
Print at 100% on A4 or US Letter. Laminate the clip-up chart and the traffic light so they last; print the data sheets fresh each week.
