These free maintenance log templates give you 20 Word documents on US Letter and A4: a maintenance log, an equipment maintenance log, a forklift inspection checklist, a vehicle inspection checklist and a safety checklist, each in colour and in black and white. I’m Patrick Elliott, a teacher and Inclusion Lead. I built these as plain, clear record sheets: one line per job with the next due date, and inspection checklists split into checks with the engine off and on.
In this guide, you’ll find which log or checklist suits each job, how to record work and inspections, and what to do when something fails a check.

What Is a Maintenance Log?
A maintenance log is a dated record of the work done on a building, a machine or a vehicle: what was done, who did it, what it cost and when it is due next. Daily inspection checklists sit alongside it, so problems are spotted, reported and fixed before they become bigger ones.
Which Maintenance Log Should You Use?
- Maintenance log: date, equipment or area, work done, done by, cost, next service due and an OK box, for a whole site.
- Equipment maintenance log: one machine per page, with its model, serial number and warranty, and its full service history.
- Forklift inspection checklist: visual checks with the engine off and operational checks with it on, each OK or needs repair, with comments.
- Vehicle inspection checklist: checks outside the vehicle, inside and while driving, with the result and signatures.
- Safety checklist: a workplace walk-round of exits, fire safety, first aid and equipment.
- Black and white versions: every template also comes in a printer-friendly black and white version with grey headings, to save ink.
How to Use a Maintenance Log
- Fill in the site or equipment details at the top.
- Write every service or repair on the day it is done, with who did it and the cost.
- Write the next due date so the following service is not missed.
- On an inspection, tick each item, write a comment for anything that needs repair and report it.
These are plain record sheets: follow the manufacturer’s manual and your local safety rules for what to check and how often. If a check fails, tick Do not use and take the equipment out of service until it is repaired.
Black and White Versions
Every template on this page also comes in a black and white version with the same layout: grey headings and an outlined title, so it prints clearly on any printer and saves ink. In the download box, choose Colour, Black and white or All, then your paper size: A4, US Letter or both.
How to Print at the Right Size
Print at 100% on US Letter or A4 and keep a pad of inspection checklists with the equipment. The black and white version copies well.
