These free bill of lading template downloads give you 16 Word documents on US Letter and A4: a bill of lading, a packing slip, shipping labels and a delivery receipt, each in colour and in black and white. I’m Patrick Elliott, a teacher and Inclusion Lead. I built these with the boxes a carrier expects on a short-form bill of lading, and matched the packing slip, labels and delivery receipt to it so one shipment has one set of papers.
In this guide, you’ll find which form goes with each shipment, what each section of a bill of lading means, and how the forms work together.

What Is a Bill of Lading?
A bill of lading is the document that goes with freight from the shipper to the consignee. It lists who is sending and receiving the goods, the carrier, what is being shipped and its weight and class, and it is signed when the carrier collects the goods and again on delivery.
Which Shipping Form Should You Use?
- Bill of lading: a straight, short-form bill with shipper, consignee, carrier, PRO and seal numbers, handling units, packages, weight, description, NMFC number and class, freight charges and three signatures.
- Packing slip: goes inside the box: order and tracking numbers, from and ship-to addresses, and items ordered, shipped and on back order.
- Shipping labels: two large labels per page with a from address, a ship-to address and the order number, weight and package count.
- Delivery receipt: proof of delivery, with the condition of each item on arrival and the receiver’s signature.
- Black and white versions: every template also comes in a printer-friendly black and white version with grey headings, to save ink.
How to Fill In a Bill of Lading
- Give the shipment a BOL number and write the date, carrier and PRO number.
- Fill in the shipper and consignee names, addresses and phone numbers.
- List each line of goods with its handling units, packages, weight, description and class.
- Tick how freight charges are paid, then sign with the driver when the goods are collected.
Check with your carrier first: some carriers ask for their own bill of lading form. Keep a signed copy of every bill, packing slip and delivery receipt together. These are general shipping forms, not legal documents for every route.
Black and White Versions
Every template on this page also comes in a black and white version with the same layout: grey headings and an outlined title, so it prints clearly on any printer and saves ink. In the download box, choose Colour, Black and white or All, then your paper size: A4, US Letter or both.
How to Print at the Right Size
Print at 100% on US Letter or A4, one copy for the shipper, the carrier and the consignee. Cut the shipping labels along the dashed line.
