These free communication log templates give you 16 Word documents on US Letter and A4: a communication log, a parent contact log, a parent communication log for one student and a phone message log, each in colour and in black and white. I’m Patrick Elliott, a teacher and Inclusion Lead. I log every parent contact I make, so I built these with the columns that matter: the reason, the outcome and the next step, kept apart so nothing is left open.
In this guide, you’ll find which log suits each kind of contact, what to write in each column and why the outcome matters as much as the date.

What Is a Communication Log?
A communication log is a dated record of every contact with a person or family: when it happened, how (phone, email, meeting), why, what was agreed and what happens next. Teachers keep one for parent contact, and offices keep one for clients and calls, so nothing agreed is forgotten.
Which Communication Log Should You Use?
- Communication log: date, time, contact name, method, reason, outcome and follow-up for any kind of contact.
- Parent contact log: every parent contact for a class in one place, with the student and the parent or guardian.
- Parent communication log: one student per page, with both parent contacts at the top and a done box for each next step.
- Phone message log: who called, for whom, the number, the message, who took it and a box for when it was returned.
- Black and white versions: every template also comes in a printer-friendly black and white version with grey headings, to save ink.
How to Keep a Communication Log
- Write each contact on the same day, while you remember the details.
- Note the method and the reason in a few words.
- Write the outcome separately from the next step, so an open issue never looks finished.
- Tick or date the follow-up when it is done.
Keep entries short, factual and polite: a parent or manager may read them later. Store logs with names securely, and follow your school’s or company’s rules on records. A4 files use UK wording, such as parents’ evening.
Black and White Versions
Every template on this page also comes in a black and white version with the same layout: grey headings and an outlined title, so it prints clearly on any printer and saves ink. In the download box, choose Colour, Black and white or All, then your paper size: A4, US Letter or both.
How to Print at the Right Size
Print at 100% on US Letter or A4 and keep one log in your planner or by the office phone.
