These free intervention log templates give you 6 Word documents on long bond (8.5 x 13 in) and A4: a class intervention log, a learner progress tracker and a weekly remediation and enrichment planner form. I’m Patrick Elliott, a teacher and Inclusion Lead. I keep a log like this after every lesson, so I built these with equal rows, a clear status key and space for the evidence that shows progress.
In this guide, you’ll find which log suits your follow-up, how to record progress with evidence, and how to plan support groups each week.

What Is an Intervention Log?
An intervention log records the extra help learners get after a lesson: who needed it, what the gap was, what you did and what changed. It is the Ways Forward step of an ILAW lesson plan, and it gives you evidence for learner progress monitoring, including the ARAL program. DepEd does not prescribe one national format, so these are independent templates you can adapt.
Which Intervention Log Should You Use?
- Class log: date, learners, competency or gap, evidence, action (remediate, reteach or enrich) and the result, for the whole class.
- Learner progress tracker: one learner from baseline pre-test, through dated sessions with check-in results and status, to the post-test.
- Weekly planner: remediation, reteach and enrichment groups with learners, competency, activity, materials and when.
How to Use an Intervention Log
- After each session, note the learners who need more help, from your formative checks.
- Write the gap as a competency, and the evidence you saw.
- Plan the action: remediate, reteach or enrich, and when it will happen.
- Record what changed, and use the status key (IP, SP, M) to track progress over time.
Keep entries short and dated: a line of evidence is worth more than a paragraph. Bring the learner progress tracker to parent meetings and ARAL check-ins.
How to Print at the Right Size
Print at 100% on long bond (8.5 x 13 in) or A4. The class log and weekly planner print landscape; the tracker prints portrait.
