These free disciplinary action form templates give you 16 Word documents on US Letter and A4: a disciplinary action form, an employee write-up form, a verbal warning form and a performance improvement plan, each in colour and in black and white. I’m Patrick Elliott, a teacher and Inclusion Lead. I built these from the steps most workplaces follow, with tick boxes for the common issues, room for the facts and a clear line that signing means receipt, not agreement.
In this guide, you’ll find which form suits each step of the disciplinary process, what to write in each section, and how to keep the record fair and clear.

What Is a Disciplinary Action Form?
A disciplinary action form is a written record of a problem at work and what was done about it: the type of issue, what happened, any earlier warnings, the action taken and what must improve. The employee signs to show they received it, which does not mean they agree, and both sides keep a copy.
Which Disciplinary Form Should You Use?
- Verbal warning form: a record of the first, spoken warning, with what was discussed, the improvement expected and a follow-up date.
- Employee write-up form: a first, second or final written warning, with what happened, what must change and what happens if the issue continues.
- Disciplinary action form: the full record, with the type of violation, previous warnings, the action taken (from a verbal warning to termination, or dismissal on the A4 version), an improvement plan and a witness signature.
- Performance improvement plan: areas to improve with the expected standard, the support given, review dates, check-in notes and the outcome.
- Black and white versions: every template also comes in a printer-friendly black and white version with grey headings, to save ink.
How to Fill In a Disciplinary Action Form
- Write down the facts as soon as you can: dates, times, places and what was seen or said.
- Tick the type of violation and any previous warnings for the same issue.
- Meet the employee, explain the action taken and agree what must improve and by when.
- Give the employee room to comment, then both sign and keep a copy each.
Stick to facts rather than opinions, and treat everyone the same way for the same issue. Follow your own disciplinary policy and local employment law, and talk to HR or an adviser before a suspension or termination.
Black and White Versions
Every template on this page also comes in a black and white version with the same layout: grey headings and an outlined title, so it prints clearly on any printer and saves ink. In the download box, choose Colour, Black and white or All, then your paper size: A4, US Letter or both.
How to Print at the Right Size
Print at 100% on US Letter or A4. Use the black and white version when you print a stack of forms for the office.
FAQs

Who made these disciplinary action form templates?
Patrick Elliott, a teacher and Inclusion Lead, designed and built every disciplinary action form by hand in Microsoft Word.
Does the employee agree with the form when they sign it?
Not necessarily. The form says that signing confirms the employee received it, not that they agree, and they can add their own comments.

A verbal warning is spoken, usually as the first step, with a short record kept on file. A written warning is given to the employee on paper and signed, usually if the issue continues.
